New Woodstock Field Office

The Visiting Nurse & Hospice of Vermont and New Hampshire has opened a new field office in Simmons House, adjacent to the Ottauquechee Health Center. The VNA& Hospice maintains field offices in many of the communities it serves, and also operates administrative offices in West Lebanon, NH, and Bellows Falls. Field offices are not used to see patients, but are small sites where nurses and therapists can pick up supplies, make phone calls and meet with team members. Other field offices are currently located in Bethel, Bradford, Brattleboro, Chelsea, Chester, Londonderry, Ludlow, Wilmington and Windsor. Some New Hampshire field offices are slated to open soon.
The Visiting Nurse and Hospice of Vermont and New Hampshire provides home care coverage to over 3500 square miles in the twin states. With a legacy of over one hundred years of service, the agency has significant depth in resources. Teams of nurses, therapists, personal care assistants, social workers, chaplains and volunteers provide homecare and hospice services in every community they serve. Last year the agency made 150,000 visits to more than 6000 clients.
The VNA & Hospice of VT and NH has recently adopted a national best practice, which has improved response time and provides continuity of care when a patient moves from homecare to hospice. This integrated nursing model means that all nurses are cross-trained to care for homecare and hospice patients. Since instituting this change, patient satisfaction scores, as measured by an independent polling company, have never been higher. Providing the highest quality care is also made possible through technology. Clinicians have specialized laptop computers that create a “virtual office,” with access to securely encrypted electronic patient records. VNA & Hospice staff is also available 24 hours a day, 7 days a week to care for patients and families.
